Tailored Outreach and Engagement: Our outreach and enrollment efforts extend further than the confines of traditional healthcare. External outreach focuses on actively seeking out uninsured people in the community, helping them find health coverage, and connecting them to care at the health center.

Enrollment Support: Enrollment assistance at our community health centers goes way beyond just the application—it’s an opportunity for empowerment and self-determination and to look at each individual as a whole. We provide holistic support to individuals seeking to enroll in health insurance and other assistance, taking into account their unique needs, preferences, and circumstances. Whether it’s navigating insurance options, accessing financial assistance programs, or connecting with community resources, we are committed to ensuring that every individual receives the support they need to thrive.

CACs and Navigators: The health centers play a vital role in helping Nebraskans understand their options for health insurance and assistance in the state. They have Certified Application Counselors (CACs) and Navigators (also known as Assisters) on staff who provide free application and enrollment assistance and education on financial assistance programs. The one-on-one attention provided by the Assisters ensures that individuals know the options available for their budget, health, and future. Assisters are trained in:

  • Health Insurance Marketplace/ ACA (Affordable Care Act)
  • Medicaid/Children’s Health Insurance Program (CHIP)
  • SNAP (food stamps)
  • General Assistance
  • Sliding fee scale (income-based)
  • Medicaid Unwind and Returning to Regular Operations

HCAN’s Outreach and Enrollment program offers trainings, technical assistance, and consultation to the health center’s Outreach and Enrollment Programs by equipping Assisters with the information needed to best meet the needs of their communities.
